Health and wellness are important for academic success during college. Students and parents are encouraged to familiarize themselves with the following information, policies and resources:
Roosevelt University is pleased to partner with The Maksin Group as our student health insurance administrator for student health insurance coverage. The Maksin Group works with a network of local and national health care providers so students have access to the best care available. Participation in the student health insurance plan is:
The annual student health insurance fee is charged to fall semester students and the spring/summer student health insurance fee is charged to new spring semester students. Students have two options:
1) enroll in the Roosevelt student plan or
2) waive the insurance because they have equivalent coverage.
The charge will appear on your student account until you waive before the advertised deadline. You must successfully complete the online waiver application to be successfully waived from this requirement. Visit www.studentinsurance.com/Schools/IL/ROOSEVELT/ to complete the online waiver application. If you do not complete the waiver or you are not approved by the deadline, you will be automatically enrolled in the plan. The application process may take up to business 10 days.
Enrollment and waiver periods end a few weeks after the start of each term. The deadline to enroll for summer is June 9, 2013 (note: College of Pharmacy students have a different deadline - refer to the insurance plan website for information). For more details about the plan benefits available to enrolled Roosevelt students, visit www.studentinsurance.com/Schools/IL/ROOSEVELT.
Optional dental insurance is also available to degree seeking students. To enroll in the dental plan, visit www.studentinsurance.com/Schools/IL/ROOSEVELT Once you're on the Maksin website, click "Enroll” and then click “Dental" to begin the process and learn more about the program. Students who enroll voluntarily pay for their insurance directly to The Maksin Group.
Student health insurance brochures are available in Auditorium Room 358 or Schaumburg Room 125. If you have questions about health insurance or you need assistance with your Roosevelt student health plan, please contact Student Services at 312-341-2004 or visit Auditorium Room 358.
Roosevelt University does not have a student health center on either campus. However, Michigan Avenue Immediate Care (MAIC) is a health facility located near the Chicago Campus at 180 N. Michigan Avenue, Suite 1605. MAIC offers walk-in treatment for immediate medical concerns but not life threatening emergencies. MAIC also has diagnostic services such as x-rays and laboratory tests, and offers medical examinations (including appropriate immunizations and prophylactic medications) for those students who might be pursuing overseas travel. The staff includes full time physicians and medical assistants. Appointments are not needed.
Students are responsible for making payment arrangements directly with MAIC. MAIC accepts most major medical plans including Roosevelt’s student health insurance program as well as cash, Visa and MasterCard. Please note that MAIC is an independent entity that is not affiliated with Roosevelt University. Always remember: for life threatening emergencies, immediately call 911.

Students are required, by Illinois law, to have an immunization record on file if they were born on or after January 1, 1957, and are planning to enroll in six or more credit hours at any time during their attendance at Roosevelt University. This record must contain proof of immunity to measles, mumps, rubella, and diphtheria/tetanus. A second measles immunization with live attenuated virus vaccine is to have been given at least 28 days after the first measles immunization. You must provide these immunization records only once during your enrollment at Roosevelt. Students who do not have immunization records to verify these vaccinations should immediately contact their physician to obtain a copy of their records or have the appropriate immunizations administered.
Download the immunization form and instructions.
The completed immunization record should be returned to the Office of Admission, 1400 N. Roosevelt Blvd., Schaumburg, IL 60173. If you have immunization questions, please call the Office of Student Services at 312-341-2004.
