Instructions for Filing a Certificate Application

If you have passed all required state tests for your content area (Basic Skills, APT, and content area) you will receive your temporary teaching certificate number by entitlement from Roosevelt University via the Illinois State Board of Education OTIS system.  The Illinois State Board of Education will send your official certificate in the mail shortly following this.  In order for Roosevelt University to process your certification, we will need the following from you:

• Completed Application for Certification Through Entitlement form (PDF, 35K).
• One copy of all official transcripts.
• A passing grade for student teaching listed on your transcript.  Undergraduates will need their degree posted.
• Please note, all incomplete submissions will be delayed.

If you have not yet passed all required state tests to complete certification, we will process your application at Roosevelt University, then we will hold your application until you notify us that you have completed the tests.  Do not send any check or money order to Roosevelt University.  We will still need:

• Please note, all incomplete submissions will be delayed.

Once we have received all of the required documentation and have let the state know that you are entitled to be certified, we will notify you via e-mail.  Instructions for how to complete the application process will be attached to the email.  You will need a credit card in order to complete the application process online.

Please do not attempt to use the OTIS system until you have been notified that your entitlement notice has been entered.  To do so may delay your certification for as long as 8 – 10 months!

It is very important to notify me immediately of any change in your email address since this is how you will be notified of deficiencies or of submission of your entitlement notice.
